The following health forms must be completed and returned to the health office no later than August 27, 2010.

1. Blue Emergency Card (will be mailed to you) Mandatory annually for all students.

2.  Health History Questionnaire (Part A) & Physical Evaluation Form (Part B)
Annually for all students: athletes and non-athletes

3. Immunization Form  New incoming 7th and 9th Graders only -
Returning students need to submit any updates to their immunization record.

4.  Dental Examination Report of Family Dentist
New incoming 7th and 9th Graders only

5.  Medication Administration: Annually by all students
consists of 3 pages (letter from the nurse/permission slip for over the counter medication/authorization to dispense prescription medications) - only submit page #2 unless prescription medication is necessary to be given during school hours

6. Villa Walsh Academy Permission Slip with Medical Release
For overnight field trips only (for example JSA trips)

The following 3 forms  have to be completed for students who have allergic reactions that require medication:

8.    Parents Consent for Student's Self-Administration of Medication
9.  Physician Certification of Student's Self-Administration of Medication

In the interest of uniformity, the Sports Physical Parent Questionnaire and the Doctor Physical Exam Form are being used as the required entrance physical and the athletic physical. Annual physicals are required for all students participating in athletics. The physicals are good for exactly one year from the date of the physical exam. Please, plan your doctor’s appointment for a physical accordingly (after July 1, 2008) or take advantage of the physical being offered by the school physician date TBA 2008.

New students: Health records from your daughter’s present school must also be forwarded to Villa Walsh Academy directed to my attention. According to New Jersey State Law, no student can be admitted to school if the above information is not on file.

In addition, athletes will need the following:

Summer Sports Camp Registration Form (camp participants only)
