VWA athletes will need to fill out FORMS 1 through 4 for EVERY SCHOOL YEAR.
Note, that Form #4 is a new form (concussion consent form)
Athletes will NOT be able to practice unless all of the forms are completed by the start dates for the specific sports.
ALL FORMS NEED TO BE SENT TO THE HEALTH OFFICE.

ANNUAL ATHLETIC PRE-PARTICIPATION PHYSICAL EXAMINATION FORM
Consists of:
Part A: HEALTH HISTORY QUESTIONNAIRE-Completed by the parent and student and reviewed by examining provider
Part B: PHYSICAL EVALUATION FORM-Completed by examining licensed provider with MD, DO, APN or PA
This form (Part A+B) is the same as the admissions physical
