VWA athletes will need to fill out FORMS 1 through 4 for EVERY SCHOOL YEAR AND FORM 5 prior to the beginning of each sport season.
Note, that Form #4 and #5 are new forms (concussion consent and Pre-participation form)
Athletes will NOT be able to practice unless all of the forms are completed by the start dates for the specific sports.
ALL FORMS NEED TO BE SENT TO THE HEALTH OFFICE.

ANNUAL ATHLETIC PHYSICAL EXAMINATION FORM
Consists of:
Part A: HEALTH HISTORY QUESTIONNAIRE-Completed by the parent and student and reviewed by examining provider
Part B: PHYSICAL EVALUATION FORM-Completed by examining licensed provider with MD, DO, APN or PA
This form (Part A+B) is the same as the admissions physical
Form-4 - Concussion consent form - NEW FORM
Form-5 - Athletic Pre-participation Form (Required no earlier than 60 days prior to the start of each sport) - NEW FORM
