To request an official transcript, please complete the Transcript Request Form in full. 

Be sure to include your full name (including maiden name, if applicable), graduation year(if applicable), current contact information, and the name and address or email of where your transcript should be sent.

Please note that official transcripts can only be sent to outside colleges, organizations, or business, not you directly. Unofficial transcripts can be sent to the individual.

Please allow adequate processing time for all requests. Incomplete forms may delay processing. If you have any questions, please contact the Guidance Office for assistance.
